Welcome to Brunswick PT! We are excited to help you begin your journey to recovery. Here’s what to expect during your first visit:

What to Bring

  • Completed paperwork

  • Insurance card(s)

  • Picture ID

  • Referral or prescription

  • Comfortable clothing (e.g., shorts for lower extremity visits)

What Happens During Your Visit

Your first visit will include a thorough evaluation by one of our skilled therapists. We’ll review your medical history and create a personalized treatment plan to help you meet your goals.

Accepted Insurances

If your insurance provider is not listed, please reach out to us, and we’d be happy to verify your coverage and benefits. When scheduling your first appointment, we kindly request that you provide your insurance information for verification. Some insurance plans may have restrictions on specific treatments, so we recommend you check your coverage.
 

As a courtesy, we file claims for your primary and secondary coverage. You will be responsible for any cost share (if applicable) after your insurance has processed the payment. Please note that some plans may have a deductible or co-pay. Payment is due at the time of service, and we accept cash, check, debit, and credit card.
